Lab Appointments

Andrology (male-factor fertility) services are available by doctor's orders only. To request a semen analysis or male infertility evaluation, please contact your urologist, your ob/gyn, or your primary care physician.

When you come for your Lab Appointment, please remember to bring the following

• Driver's license or State ID
• Semen Test Requisition Form

Sugar Land - 8:30 am to 10:30 am

Webster - 8:30 am to 10:30 am

Katy - 7:30 am to 12:30 pm

Physician Appointments and Consultations

Reproductive endocrinologists are available by appointment through Fertility Specialists of Houston, at 713 512 7900. Appointments are available at Texas Andrology Services and other locations.

Please arrive 10-15 minutes early for your Appointment for registration and to fill in a health questionnaire.

When you come for your Appointments, please remember to bring the following

• Driver's license or State ID
• Insurance information
• Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits
• Medications list (if any)
• If you have had surgery elsewhere, please bring a copy of your operation report

To cancel an Appointment

Telephone the office during business hours and allow at least 1 day's notice so that we can offer your appointment time to patients on our waiting list.

We recognise that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologise if we keep you waiting.
